Background: Metal Penile ring entrapment during sexual self-satisfaction is one of the rare conditions in urological surgery. When the shaft of the penis becomes trapped in a metal ring, one risks losing their penis completely due to the lack of blood flow. We present the management of a case of an entrapped penis ring in the absence of management guidelines.

Methods: A 61-year-old male presented after 24 hours of very thick ring penile insertion for the sustainability of an erection, with painful penile swelling and acute urine retention. The patient was suffering from erectile dysfunction. Many failed attempts lasted for more than 5 hours due to the thickness of the metal ring under general anaesthesia. The iron saw and the use of metal wire were used. In addition to Orthopaedic tools were. But ultimately the hospital maintenance crew was called and they used a special iron cutter to break the metal ring.

Results: The entrapped penile ring is clinically diagnosed but it is difficult to determine the correct method to remove it. Entrapped ring obstructs blood and lymphatic flow leading to edema and ischemia.

Conclusions: the use of a metal ring for the penis must be revaluated and scientific discussion should be made about the harms of continuous to sued such cases is a fruitful addition to doctors to know the best way to deal with the problem with severe lack of guidance methods.
